[Youngadultservices] Teen field trip question

We are going to try to do a Teen aspect to our SRP this year and I
have been tossing around the idea of taking the teen participants to see a
Rockford Riverhawks game. What are the liability issues for this? For
this we would be counting on volunteers for transportation. Do other
libraries take group of children/teens on field trips? Do they have a
permission slip/waiver template?
